PHONE ORDERS |

Call 1-866-478-3417.
A Case
Manager will be assigned to you.
-
He/she
will give your Requisition Number and Login
information for your report.
-
This
process takes about 5 minutes.

ONLINE ORDERS |
| |
- You
will receive an email with your order ID Number.
- A
Case Manager will be assigned to you. He/she will provide a
Requisition Number for your
Service Center visit
and Login information for your report. Emails and
Login are encrypted and password protected. We also fax a
copy of your testing Document to the Service Center.
- Call
your assigned Case Manger 30 minutes prior to your visit to
review your case and have all your questions answered.

2.
PHONE &
ONLINE ORDERS |

- Go to
the Service Center and bring
the Requisition Number with you.
- A
certified technician will collect your specimen (blood
and/or urine sample) - The visit usually takes 15
minutes.

Login, see and print your report. We can also fax or mail
the report to you.

Reports are usually available in 2-3 business days.
- Your
Case Manager, a Physician authorized representatives and
our Physician are available for test review and to make sure
results are understood and all your questions are answered.
